I am constantly receiving praise at work. How do I ask for a raise/promotion?

I have been in my position for over two years and have received extra responsibilities, but I do not believe that my current pay is matching up with my experience. I have grown so much in my position and would like to receive a promotion/raise that reflects my growth.
